Registering as a Patient
If you would like to register with the practice, please visit the practice with two forms of ID to complete our registration forms.
New Registrations Check
All newly registered patients are advised to attend for a health check with the Practice Nurse, this will provide details of your medical history from the start.
Children under the age of 5 years do not require a health check but parents/guardians should be able to provide immunisation details.
Patients living outside of our Practice Area
Patients who reside outside of our Practice Area may still register with our surgery if they wish however they must be aware that certain services are not available to patients living in other areas such as Community Services. We are also unable to provide home visits to these patients. Patients should consider this before registering with the Rowans Surgery.
On occasion we may receive notification from the Primary Care Support service that you are no longer in the area therefore we must de-register you from the practice list. In this instance we will endeavour to contact you before removing your registration. If this fails you will be de-registered. You can re-register with the Practice at any time by completing and submit a registration form.
Registering your New Baby
You need to register your new baby at the practice as soon as possible, so please complete our online registration. Please ensure you provide your baby’s NHS Number which will be detailed in their Red Book. Once we have received the form we will register your baby as a patient at this Practice.
The Practice will send you an appointment for a mother and baby post-natal check. This is a 50 minute appointment, 30 minutes for the baby check and 20 minutes for the post-natal. Your baby’s course of routine immunisations will start at 8 weeks and will be given at this appointment.
Please remember to bring the red book.
If you cannot make this appointment you must notify reception as soon as possible. Please be advised that if you do need to cancel, the next available appointment may be anything up to 4 weeks after that date.
The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.
To register you will need to complete our Temporary Resident Registration form, and bring in your passport when coming into the surgery.
